Πέντε λόγοι για τους οποίους το τείχος ανοσίας είναι δύσκολο να επιτευχθεί

Όσο ο εμβολιασμός του πληθυσμού έναντι του SARS-CoV-2 προχωρά, τόσο δημιουργούνται ερωτήματα για το αν η πανδημία αυτή θα λήξει με το χτίσιμο του «τείχους ανοσίας». Το όριο του τείχους ανοσίας είχε τοποθετηθεί στο να επιτευχθεί ανοσία σε 60-70% του πληθυσμού είτε μέσω νόσησης, είτε μέσω εμβολιασμού. Αυτός ο στόχος όσο διανύουμε το δεύτερο έτος της πανδημίας φαντάζει δυσεπίτευκτος, λόγω των νέων στελεχών, του σκεπτικισμού απέναντι στον εμβολιασμό και του καθυστερημένου εμβολιασμού των παιδιών.

Σύμφωνα με δημοσίευση στο περιοδικό Nature, το γεγονός ότι το τείχος ανοσίας είναι δυσεπίτευκτο σενάριο πλέον δεν υποβαθμίζει την αξία του εμβολιασμού ως όπλου απέναντι στην πανδημία. Στο άμεσο μέλλον οι επιστήμονες πλέον θεωρούν ότι η επιστροφή στην κανονικότητα δεν προϋποθέτει το τείχος ανοσίας, αλλά μακροπρόθεσμα η λοίμωξη COVID-19 θα έχει ενδημικό χαρακτήρα. Το σκεπτικό αυτό βασίζεται σε διάφορους λόγους, όπως:

Είναι ασαφές αν και κατά πόσο τα εμβόλια προφυλάσσουν από τη μετάδοση του ιού. Κλειδί για το τείχος ανοσίας είναι ότι και αν νοσήσει κάποιος είναι λίγα τα επίνοσα (που μπορεί να μολυνθούν) άτομα στο περιβάλλον του. Τα εμβόλια έχουν αποδείξει την αποτελεσματικότητά τους έναντι στη συμπτωματική νόσο, αλλά τα δεδομένα γύρω από την πρόληψη της μετάδοσης δεν είναι ώριμα ακόμη, και αυτό αποτελεί κομβικό σημείο στο να συμβάλει ο εμβολιασμός στο τείχος ανοσίας. Δεν απαιτείται προστασία 100% από τη μετάδοση, και ένα ποσοστό της τάξης του 70% προστασίας του εμβολίου από τη μετάδοση του ιού θα ήταν επαρκές.
Ο διαμοιρασμός των εμβολίων είναι άνισος. Μία συντονισμένη ταυτόχρονη παγκόσμια εμβολιαστική κινητοποίηση θεωρητικά θα μπορούσε να εξαλείψει τη λοίμωξη COVID-19. Ωστόσο, η διαφορετική εμβολιαστική κάλυψη ανά τις χώρες, ή ακόμα μέσα στις πολιτείες της ίδιας χώρας (ΗΠΑ), και οι διαφορετικές ηλικιακές ομάδες που εμβολιάζονται ανά συγκεκριμένη περίοδο, με τα παιδιά να μην εμβολιάζονται ακόμη, μπορεί να οδηγήσουν σε νέες εξάρσεις ακόμη και σε χώρες με ποσοστά εμβολιασμού που αγγίζουν το τείχος ανοσίας. Αν υποθέσουμε ότι οι περισσότεροι ανήλικοι δεν εμβολιαστούν (μιας και δεν έχουν εγκριθεί ακόμη τα εμβόλια για ηλικίες κάτω των 16 ετών), τότε πρέπει να εμβολιαστεί το 100% του πληθυσμού άνω των 18 ετών, για να φτάσουμε ανοσία 76% στο σύνολο του πληθυσμού.
Τα νέα στελέχη ανατρέπουν την ισορροπία του τείχους ανοσίας. Το παράδειγμα της Βραζιλίας εξηγεί αυτή την περίπτωση. Λόγω των μαζικών εξάρσεων στην αρχή της πανδημίας έως τον Ιούνιο του 2020 υπολογίστηκε ότι νόσησε πάνω από το 60% του πληθυσμού, ποσοστό που θα μπορούσε να πετύχει το τείχος ανοσίας. Ωστόσο τον Ιανουάριο του 2021, το νέο στέλεχος, γνωστό και ως Ρ.1, οδήγησε σε νέα έκρηξη κρουσμάτων. Ο ταχύς μαζικός εμβολιασμός θα μπορούσε να αποτρέψει τα νέα στελέχη από το να εμφανιστούν και να κυριαρχήσουν.
Η ανοσία μπορεί να μη διαρκεί για πάντα. Το τείχος ανοσίας της επιτυγχάνεται είτε μέσω του εμβολιασμού είτε μέσω της φυσικής λοίμωξης. Το χρονικό διάστημα, που διαρκεί η ανοσία που παρέχει η φυσική λοίμωξη δεν είναι γνωστό. Τα έως τώρα δεδομένα από προηγούμενους κορωνοϊούς δείχνουν ότι η ανοσία σταδιακά μειώνεται, δεν φτάνει στο μηδέν αλλά δεν είναι για πάντα στο 100%. Επίσης τα εμβόλια δεν είναι αποτελεσματικά σε ποσοστό 100%. Αν υποθέσουμε ότι η φυσική ανοσία διαρκεί λίγους μόνο μήνες, τότε τα χρονικά περιθώρια για την εφαρμογή του εμβολιασμού είναι ακόμα πιο στενά. Επίσης πρέπει να προσδιοριστεί η διάρκεια της ανοσίας που παρέχουν τα εμβόλια και πότε και αν θα χρειαστούν δόσεις ενίσχυσης.
Η ολοκλήρωση του εμβολιασμού αλλάζει την ανθρώπινη συμπεριφορά. Όσο οι άνθρωποι εμβολιάζονται, σταδιακά χαλαρώνουν τα μέτρα πρόληψης, αυξάνουν τις συναναστροφές τους και γέρνουν τη ζυγαριά του τείχους ανοσίας, το οποίο εξαρτάται και από την έκθεση του ατόμου στον ιό. Τα εμβόλια έχουν αποτελεσματικότητα που αγγίζει το 90%, οπότε αν πριν το εμβόλιο κάποιος ερχόταν σε επαφή με ένα άτομο, και μετά έρθει σε επαφή με 10, τότε καταλήγει στο ίδιο σημείο. Στη διακοπή της αλυσίδας της μετάδοσης θα συνεισφέρει η χρήση μάσκας, εμποδίζοντας την επικράτηση νέων στελεχών μέχρι ο εμβολιασμός να ολοκληρωθεί. Οι άνθρωποι πολύ γρήγορα επιθυμούν να επιστρέψουν στη ζωή και τη συμπεριφορά τους πριν την πανδημία, ωστόσο είναι σημαντικό να γίνει σαφές ότι δεν είμαστε ακόμη ασφαλείς από την πανδημία, αλλά είμαστε «πιο ασφαλείς από ό,τι ήμασταν».

Με βάση τα μέχρι τώρα στοιχεία ο εμβολιασμός μπορεί ρεαλιστικά να μη μας παρέχει το τείχος ανοσίας και να μην μπορεί δώσει άμεσο τέλος στη λοίμωξη COVID-19, αλλά μπορεί να μειώσει τις νοσηλείες και τη θνητότητα της λοίμωξης ειδικά σε ευπαθείς ομάδες. Η λοίμωξη μπορεί να μην εξαφανιστεί άμεσα, αλλά η σοβαρότητα και η απειλή της θα εξασθενήσουν.
